New Student Registration

What you need to register your child

Items you will need to bring with you to the front office to register your child are

  • Proof of residency 
    • This can be a water, electric or gas bill.
    • This can be a notarized letter from your landlord.
    • This can be a notarized letter from whomever you are living with that states you live in Sequatchie County.
  • Birth Certificate of the child
  • Social Security Card of the child
  • TN certificate of Immunization
  • Physical Examination
    • Both the immunization and physical received can be on the same white page that you receive from your doctor or from the health department.
  • Custody Papers (if applicable)

 

There will be forms you need to fill out when you come to register your child

  • Enrollment Card
  • Home Language Survey
  • Migrant Survey
  • Request for Record (if applicable)
